TA
Trusted Assessment (TA) Process is used for a case can be referred on to other services when you onward refer a service user.
Here is the process when requesting a TA / Intergration work:
Complete MHST Initial Assessment with CYP/Caregiver, including ROMs, formulation and Risk Assessment.
Practitioner discusses with supervisor and agrees next step, if the need exceeds MHST threshold.
Practitioner or supervisor contacts MHST Clinical Lead (Heleon King 07974071996) to discuss need for TA.
If agreed, practitioner phones the service they would like to refer to and requests a TA call. This should be offered within 2 weeks. (See contact in specific services here)
After the TA call was made, clinician from the receiving team will make a decision about whether the case transfers to their team. The call should be logged on PCMIS as an apoointment with notes using the Professinal Consultation option in event details.
MHST practitioner sends Discharge following trusted assessment letter to family / CYP to inform of decision made.
MHST practitioner sends a Handover Letter with copies of the IA and relevant reports to the admin of receving team within 1 week of the TA call. For referral to the targeted team you must include SDQs and the name and email address of the cureent social worker.
Log the TA /Intergration conversation and the outcome on PCMIS.
