Referral pathway & service user journey
Referrals for 1:1 interventions
Referrals for 1:1 interventions with CYP or their P/C should be made by an educational professional using the MHST online referral portal, link below. Forms should be completed using as much detail as possible for teams to assess in the most holistic and accurate way.

Group referrals
Referrals for targeted groups & requests for WSA activities (such as an assembly, workshop, or universal group) are to be made using the Group/WSA Request Form. This is not currently available on the portal, instead all referrals are agreed between school and their practitioners in the consultation meeting.
Referral meetings
Referral meetings take place once per week (day/time to be confirmed locally within each MHST). They are attended by the Team Leader, Senior Practitioners and Admin staff. These meetings will provide dedicated time for peer discussion and triage of referrals. They are stored on each team’s Shared Drive in their Practice Governance folder. They will also provide time for Workbook updating
If/when a referral is accepted, then the service user’s journey will begin on the MHST 20-week Referral Target Pathway, and they will move onto the assessment and allocation phase. Referrals will also be discussed weekly with an EMHP, during their CMS, following the referral meeting, with subsequent feedback also given to the referring school/college.
A letter will be sent to inform P/C or CYP (if over 16yrs) of acceptance to the MHST service via email. This letter is the MHST Intro Letter found on PCMIS under letters. The letter is a template that has been pre-populated and can be completed by the Practitioner/Admin and sent out to the P/C or CYP (if over 16yrs). The referrer (educational setting) will also be cc’d into this email.
A letter will also be sent to the SU GP, via email, to inform them of the SUs acceptance to the MHST service. This letter is the GP Intro Letter found on PCMIS under letters. The letter is a template that has been pre-populated and can be completed by the Practitioner/Admin and sent out to the SU GP.
All referrals are expected to be deactivated within 20 weeks, this includes up to 8 weeks of pre-treatment time. All referrals are also expected to be triaged within 2 weeks.
If one of the above time limit was not met, it will trigger a pathway breach, which then needs to be reviewed.
