Initial Assessment (IA)
Consent to share information
Consent to share information should be discussed during the triage call (if carried out) or during the IA (Session 0). This will be recorded on PCMIS.

Next steps
At the end of the IA, the practitioner clearly explains the next steps for parent/carer/young person. The practitioner explains that the IA will be discussed with Team Leader/supervisor and sets a timescale for the follow up telephone call in which the outcome of the assessment is to be communicated. A letter to young person/parent/carer/GP to be sent following IA summarising assessment & outcome.
A discussion will be had with the SP/TL after an initial assessment and a decision will be made as to whether go ahead and offer an intervention with the MHST or whether the case is not suitable for work with the MHST.
Risk Assessment and any Safeguarding & Datix MUST be completed on the day of the IA. Session notes and all further documentation notes to be completed within 2 working days of the IA.
The SU’s GP will be notified in writing of any safeguarding risks identified that require a referral to Children’s Social Care using the GP Safeguarding letter found on PCMIS under letters. The letter is a template that has been pre-populated and can be completed by the Practitioner/Admin and sent out via email.
Case Decision
Case Suitable
Where the case is suitable for MHST intervention the EMHP will advise the P/C or CYP (if over 16yrs) by phone or email. A letter will be sent to the SU GP, via email to advise them that the SU has been taken into treatment. This is the GP Intro Letter_standard found on PCMIS under letters. The letter is a template that has been pre-populated and can be completed by the Practitioner/Admin and sent out to the P/C or CYP (if over 16yrs).
Case unsuitable
Where the case isn’t suitable for MHST intervention, the following steps should be taken:
EMHP communicates the outcome of the assessment to the P/C or CYP (if over 16yrs) and educational setting
Any suitable signposting to be offered
An onward referral can be supported

EMHP/Admin to send a GP Discharge letter to the SU GP summarising outcome. The letter can be found on PCMIS under letters. The letter is a template that has been pre-populated and can be completed by the Practitioner/Admin and sent out via email
EMHP requests another referral from the educational setting
