Ending
Once an intervention has ended, whether the full number of sessions have been completed or the intervention finished early, the following steps must be taken:
ROMs need to be repeated & recorded on PCMIS
(ESQ)
A Friends & Family Test (FFT)
An End of Treatment Letter is to be sent by the practitioner to the parent/carer/young person/GP & other agencies involved in the young person’s care such as, social services- summarising the intervention. The MHL at school will also be informed. These letters will also cover any signposting/onward referral recommendations
EMHP to add closing note & close case on PCMIS
Discharge vs Decline
Only cases turned down at the referral meeting should be marked as declined on PCMIS.
All cases taken forward to a triage call or IA should be classed as a discharge whenever they leave the pathway as they have had at least one meaningful clinical contact.
