Incident reporting System
Datix is to be completed if making a safeguarding referral or a change in risk by the practitioner. Safeguarding referral to be attached to the Datix and Datix Number must be input into PCMIS.

What do I report on Datix?
An incident is defined as an event or circumstance which could have resulted, or did result in, unnecessary damage, loss or harm to a service user, member of staff, visitor or member of the public under the care of the Trust or on Trust premises. You can also report non HPFT patient safety incidents for example if you witness an incident relating to one of our service users in a care home or other setting external to HPFT.
We have tried to compile a list of some obvious and less obvious examples. Please remember if you are made aware of something it is your responsibility to report it - we would rather something is reported twice than not at all. If an incident is repeatedly occurring and you do not feel there is an appropriate category on Datix, please contact us – we will be happy to add one.
Hints & Tips
Choose the closest incident type, category and sub-category for the incident – you can contact the Safer Care Team if you are unsure on 01727804826
Do NOT include information that could identify anyone such as names or initials in the main text boxes (Details of Incident & Immediate Action Taken) this is because we export this information to NHS Improvement for patient related incidents.
In the ‘Details of Incident’ box please include as much detail as possible about the incident and any immediately preceding events which may have led up to it – include the outcome if known i.e. if it’s an AWOL and you know the service user has returned etc. As a guide, a paragraph or two is normally sufficient.
Use ABC spell check icon when form is completed
Record the facts of the incident - please do not include your opinions about what may have happened.
It is not necessary to report incidents word for word unless it is relevant to the type of incident being reported e.g. racial abuse. If you are quoting word for word use “”.
Remember to record the names of everyone involved in the incident. Please remember to choose the appropriate role for each person involved in the incident

Reporters cannot access the incident form once submitted
Please be mindful that Datix forms may be disclosed as a result of Freedom Of Information requests from service users /carers/relatives
