MHSTs need to be made aware if any child/young person has a level of SEND need when they are referred to the service (questions to illicit this information is included on the online referral form on PCMIS) as this will help with:

• Making adaptations to any provision offered, if needed.

• Consulting with the specialist SEN MHST Service (see below).

• Making onward referrals to the service, if needed.

• An awareness of the child/young person’s targets if we are included on their EHCP. EHCPs should be uploaded as a document to PCMIS if school/college and/or parents/carers agree to provide them.
